HOME SAFE MAKES IT EASY!
- Obtain a receipt from your Uber, Lyft or taxi ride home and mail the receipt to: Lytle EAP Partners, 200 Cedar Ridge Drive, Suite 208, Pittsburgh, PA 15205
- No form needed just include your name, address, phone number, and employer on the receipt.
- Reimbursement is limited to 3 times per household, per calendar year, January through December, up to $25 reimbursed per ride, when a receipt is submitted within 30 days from the safe ride home.
- A check will be mailed out in 6-8 weeks.
