I need to add or remove a dependent to my coverage

I recently had a baby: To add a new child to your coverage, you must make a request to your School Entity’s benefits administration office within 30 days of the birth of the child. You will be required to provide the child’s name, birth date, Social Security Number and a copy of the birth certificate. Once the appropriate documentation is received and confirmed by the appropriate administrators, the child will be added to your insurance on the first day of the month following their birth date. Any additions requested after the 30-day period will be required to wait until the next open enrollment period.

Once the appropriate documentation is received and confirmed by the appropriate administrators, the child will be added to your insurance on the first day of the month following the final adoption. Any additions requested made after the 30-day period will be required to wait until the next open enrollment period.

I recently got married: To add your new spouse to coverage, you must make a request to your School Entity’s benefits administration office within 30 days of your marriage. You will be required to provide your spouse’s name, birth date, Social Security Number and a copy of your marriage license. Once the appropriate documentation is received and confirmed by the appropriate administrators, your spouse will be added to your insurance on the first day of the month following your marriage. Any additions requested made after the 30-day period will be required to wait until the next open enrollment period.

I recently got divorced: You must notify your School Entity’s benefits administration office within 60 days of your divorce being finalized that you would like to remove your spouse. When your divorce is finalized, you will need to provide a copy of the final legal divorce decree. Your spouse can only be removed when the divorce is finalized. Once the appropriate documentation is received and confirmed by the appropriate administrators, coverage will be terminated on the first day of the month following the divorce. Once the School District terminates your spouse, they will be offered coverage through COBRA.
